By completing and signing this tournament application, I/we hereby waive and release the Children’s Therapy Center Organizing Committee, the Children’s Therapy Center, its officers, sponsors, all other contestants, tournament officials, and special guests from any and all claim for injury and/or damages incurred in connection with this tournament. In addition, if I am providing the boat for this tournament, I hereby acknowledge that I have current boater liability insurance in force during the date of the tournament.
